Parents who are wanting to register their child/children at the Long Creek School District for the upcoming school year, may do so by several different ways. You can download and fill out registration forms, fill them out, and either email, mail, or turn them in at the Long Creek School Office. Please email the forms to Jennifer at [email protected] or postal mail them to Long Creek School, Attn: Registration, P.O. Box 429, Long Creek, OR 97856. Hard copies of these PDF registration forms may be picked up at the school office.
